WAIVER AND RELEASE

I hereby declare that I am the legal owner of my dog and that my dog has not been exposed to distemper, rabies, parvovirus, or any other contagious disease within the past thirty (30) days. I confirm that my dog has been vaccinated, and I will provide proof of current vaccinations via email to Paradise Kennels.

I understand that boarding my dog carries inherent risks. I authorize Paradise Kennels Ltd. to take any necessary actions for the safety, health, and well-being of my dog while under their care, including seeking professional veterinary treatment. Every effort will be made to provide a safe environment for my dog. I fully understand that I, the owner, am responsible for any injuries that occur at Paradise Kennels, as I have granted permission for my dog to engage in all activities at the facility, including but not limited to scratches, cuts, rashes, bites, foot injuries, and sprains.

If my dog requires immediate medical attention, Paradise Kennels will contact me to make arrangements. If I cannot be reached, they will transport my dog to the Emergency Animal Clinic if any injury is deemed life-threatening. I certify that all known medical and behavioral conditions are disclosed, and I will not hold the kennel liable for these conditions or for injuries due to stress-related behaviors (e.g., scratching, chewing, pacing, etc.).

If my pet is injured or becomes ill while boarded, I authorize Paradise Kennels to provide first aid or seek veterinary attention, understanding that any associated costs will be added to my invoice. I permit Paradise Kennels Ltd. staff to administer pet first aid for any injuries deemed non-life-threatening.

I am solely responsible for any damage or injury incurred while my dog is boarded, including payment for injuries to staff, other clients, and other animals, as well as for any damage to facilities caused by my dog. I waive any and all claims, actions, or demands of any nature, whether foreseen or unforeseen, that I may have against Paradise Kennels Ltd. relating to the care, control, health, and/or safety of my dog during pick-up, transportation, drop-off, and stay at the facility.

Paradise Kennels staff have the right to determine whether it is safe for my dog to participate in various activities, including but not limited to playing with other dogs, going on walks (on or off-leash), and swimming. I understand that my dog’s acceptance into boarding is at the discretion of Paradise Kennels Ltd., which reserves the right to refuse service for any reason.
If my dog has a history of or consistently demonstrates aggression or biting toward humans or other animals, Paradise Kennels Ltd. reserves the right to refuse service. By signing below, I agree to the above terms, accept full responsibility for my dog’s actions while boarding, and release Paradise Kennels Ltd., its owners, and staff from all claims of liability.

I acknowledge that I have carefully read all parts of this Agreement, understand its contents, and recognize that it includes an assumption of risk related to the negligence and liability of Paradise Kennels Ltd.

I further acknowledge that Paradise Kennels Ltd. is materially relying on this waiver and is allowing my dog(s) to engage in play with other dogs, go on off-leash walks, and use the play areas during their stay. My dog’s boarding is not without risk. I authorize Paradise Kennels Ltd. to do whatever they deem necessary for the safety, health and well-being of my dog(s) while under the care of Paradise Kennels Ltd. including seeking professional treatment for my dog(s). Every effort will be made to provide a safe environment for my dog(s).

This contract contains the entire agreement between the parties. All terms and conditions of this contract shall be binding on the heirs, administrators, personal representatives, and assigns of both the owner and the kennel. Any controversy or claim arising out of this contract, or its breach, or any claim involving alleged negligence by either party, shall be settled by arbitration.
